Facility Evaluation Report

Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to conduct a case management regarding an incident report LPA received. LPA met with staff who then contacted Administrators via telephone. LPA spoke with Administrator via telephone to explain the purpose of the visit.

During today's visit, LPA was informed that client (C1) has returned to the facility and is thriving. C1 will be having a check up with primary care physician and will be receiving physical therapy. Fall prevention has been in place.

LPA and Administrators agreed this matter will be continued after primary care physician appointment and C1's upcoming appointment if a higher level of care is needed.

Exit interview and a copy of the report was provided. Signature was received by staff as Administrators were not at the facility. Signature is acknowledge receipt of the report.
