Facility Evaluation Report
On January 21, 2026, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding the effective exclusion of an individual. LPA met with staff who then contacted Administrators via telephone. LPA explained to Administrators the purpose of the visit.
LPA and Administrators discussed the letter was received on January 20, 2026, and facility has since comply with the exclusion.
During today's visit, LPA assisted with removing S1 from Guardian roster to reflect employment accuracy.
Exit interview and a copy of the report was provided. Signature was received by staff as Administrators were not at the facility. Signature is acknowledge receipt of the report.
