Facility Evaluation Report
On December 1, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to conduct a case management visit regarding a deficiency observed. LPA met with staff and explained the purpose of the visit. Staff contacted Licensee, who arrived to the facility shortly afterwards.
The purpose of the visit was to discuss the status of facility administrator.
On November 3, 2025, LPA received an email notification from Administrator of her resignation effective October 31, 2025. LPA has not receive any new updated from Licensee of appointed Administrator. At this time, there is no active administrator on file.
As a result of facility not have an active Administrator since October 31, 2025, deficiency cited.
