Facility Evaluation Report

On December 1, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to conduct a case management visit regarding a concern observed. LPA met with staff and explained the purpose of the visit. Staff contacted Licensee, who arrived to the facility shortly afterwards.

Today's visit, it was observed that the property was recently sold in September 2025, but notification was not submitted to LPA. LPA and Licensee discussed the current status regarding the control of property. Licensee stated he does have an active lease with the landlord. It was discussed that Licensee is to have control of property until the new change of ownership application is submitted and approved.

At this time, Licensee will submit a copy of the lease agreement to LPA by Friday, December 5, 2025. Additionally, an office meeting will be scheduled to discussed change of ownership.

Exit interview conducted and a copy of the report was provided.
