Facility Evaluation Report

On January 28, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to conduct a case management visit. This is a subsequential visit following LPA's visit on December 1, 2025. LPA met with Licensee and explained the purpose of the visit.

On December 1, 2025, it was discussed with Licensee regarding the property being sold. Licensee has proof of control of property with a lease back agreement with current landlord to ensure facility can remain operable.

Today's visit, LPA and Licensee discussed Licensee's failure to notifying Community Care Licensing Division and all residents along with responsible party with a 30 day notice regarding th e transfer of the property.
