The inspector’s account

Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection when LPA observed a following deficiency. LPA met with staff and explained the purpose of the visit. Administrator was unable to make it to this visit.

Based on interview, LPA was informed today's census is three residents in care. LPA was additionally informed two residents are at the hospital for evaluation. LPA was informed R1 was sent out on April 1, 2025 and R2 was sent out on April 17, 2025. LPA was unable to locate any submitted incident reports submitted by Licensee.

Exit interview and a copy of report and appeal rights was emailed.
