Facility Evaluation Report

Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding a facility action report (FAR) completed on December 18, 2025 by Alta California Regional Center. LPA met with staff and explained the purpose of the visit. Staff then contacted Assistant Administrator who arrived to the facility shortly afterwards.

Based on FAR, staff (S1) was hired back in July 2025, but S1's fingerprint clearance has been pending via Guardian. It was discussed that individuals who work, volunteer, and reside at the facility are subject to a criminal record clearance prior to presence at the facility. LPA was informed there was a misunderstanding regarding S1's clearance but once made aware, S1 has been removed from schedule.

LPA provided Assistant Administrator a copy of facility's Guardian roster and S1's required documents for clearance.
