Facility Evaluation Report

LPA Hiratsuka, conducted this unannounced annual visit. Administrator Maria Yeldon arrived during visit.

This facility has a fire clearance for two non-ambulatory and four ambulatory residents. This facility has three resident rooms and three staff rooms. There are two full common bathrooms. The common areas were toured and no hazards observed. The backyard was inspected. There are two locked sheds in the backyard. There is an ample supply of perishable and non-perishable food supply.
