Facility Evaluation Report

On January 21, 2026, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct an annual inspection utilizing the inspection tool. LPA met with Licensee who contacted Administrator who arrived to the facility shortly afterwards.

Facility was observed to have the required posters posted in the common areas. Administrator Certificate #7004217735 was observed to be active with effective date of March 14, 2024 to March 13, 2026.

Today's inspection, LPA and Administrator conducted a tour of the interior of the facility to ensure the health and safety of clients in care. Areas toured included but not limited to: four clients bedrooms, two bathrooms, linen closet, kitchen, garage and the common areas.

LPA observed fire extinguishers present with recent service date of December 1, 2025. LPA observed facility to be at a comfortable temperature of 74*. Food supplies was observed to be adequate for three clients in care. LPA observed facility to have ample supply of linens. Clients bedrooms were observed to be clean and well kept. Bathrooms were observed to be free from odor, equipped with paper towels, soap and nonskid mats inside the shower. Sharps, toxins and medications were observed to be locked and secured. No immediate concerns observed.

File review of facility's Emergency Disaster Plan revealed it was recently reviewed and updated January 15, 2026. File review was conducted for three clients records and one personnel record. LPA observed the required documents on file. Additionally, LPA obtained a copy of facility's surety bond, LIC 308 and LIC 500. CARE tool was completed and no deficiencies cited.

Exit interview was conducted with Administrator. A copy of report was provided.
