The inspector’s account

On January 21, 2026, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with Staff and explained the purpose of the visit.

Today's inspection, LPA and Staff conducted a tour of the interior of the facility. Areas toured included but not limited to: three shared client bedrooms, bathroom, medication room, laundry room, kitchen and the common areas. LPA observed facility to have medications, toxins, and sharps to be locked and inaccessible to clients in care. LPA observed facility to have appropriate amount of food available. LPA observed facility to be safe and sanitary, no immediate concerns observed.

File review was conducted. Emergency Disaster Plan was recently reviewed and updated December 1, 2025. Fire drill record revealed facility has been conducting drills month for year of 2025 and recently conducted a drill on January 8, 2026. Water temperature check confirmed facility hot water temperature is safely at 118* degrees. Client records and staff records were observed to be complete and on file, ready for review.

LPA, Staff and Licensee discussed submitting documents to appoint new Administrator on file. LPA has reached out to Administrator Certificate Unit for a copy of Staff's certificate, once received, a copy will be forwarded to Licensee.

CARE tool was completed and found facility to be in compliance, no deficiencies cited.

Exit interview conducted and a copy of report was provided.
