Facility Evaluation Report
On October 23, 2205, Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with staff who contacted Licensee who arrived to the facility shortly afterwards.
A tour was conducted of the interior and exterior of the facility to ensure the health and safety of clients in care. Areas toured included but not limited to: clients bedrooms, bathroom, kitchen, laundry room, recreation room, staff room, backyard and the common areas. Bathrooms and bedrooms were clean and in good repair. There is a locked storage for medications and toxins. Fire extinguisher was recently serviced. In areas toured, no immediate health, safety, personal rights observed.
File review conducted for clients records and personnel records. LPA observed all required documents present. LPA observed documentation of monthly fire drills conducted. LPA observed Emergency Disaster Plan to be recently serviced.
CARE tool was completed and found the facility to be compliance.
Exit interview was conducted. A copy of this report was provided.
