The inspector’s account

On October 28, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct an annual inspection, utilizin g the inspection tool. LPA met with Licensee and explained the purpose of the visit. Administrator arrived shortly afterwards.

A tour was conducted to ensure the health and safety of clients in care. Areas toured included but not: clients bedrooms, bathrooms, kitchen, office, laundry room, staff room, and the common areas. LPA observed sharps, toxins and medications to be locked and secured. LPA observed facility to have two days of perishables and seven days of non-perishable foods present and available for clients in care. LPA observed fire extinguisher present, with service date of December 4, 2024. LPA observed fire drill records to be posted with documentation of drills conducted monthly at the facility for the year of 2025. In areas toured, LPA observed facility to be clean and in good repair. No health, safety and personal rights violations. LPA reviewed the facility's Disaster Plan to be complete. File review of clients records and personnel records was conducted, LPA observed records to be complete and available for LPA to audit. At this time, no concerns was reported to LPA.

LPA and Administrator discussed possibly needing an age exception in the future. Facility is to notify LPA when if acceptance or retention of an individual 60 years of age or older would result in the number of persons 60 years of age or older exceeding 50 percent of the census in facilities with a capacity of six or fewer clients, or 25 percent of the census in facilities with a capacity over six, the licensee must request an exception in order to accept or retain the individual. The exception request must be made in accordance with Section 80024. The documentation specified in Section 85068.4(c) must be submitted with the exception request. A copy of 85068.4 Acceptance and Retention Limitations was provided.

CARE tool was completed with Administrator and found facility to be in compliance to Title 22. No deficiencies cited. Exit interview and a copy was provided.
