Facility Evaluation Report
Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to conduct a required annual inspection. Upon arrival, LPA observed facility to be vacant. LPA contacted Administrator on file who then contacted Licensee who arrived to the facility. LPA met with Licensee, and explained the purpose of the visit.
Today's visit, LPA observed facility to have no residents in care. LPA was informed facility is currently going through a change in operation to be vendorized with Alta California Regional Center. A new plan of operation will be submitted to the Department once approved.
LPA and Licensee conducted a tour of the interior and exterior of the facility to ensure facility is still in compliance to California Code Regulation, Title 22 Chapter 8. Based on the areas toured, LPA did not observed any concerns at the facility. Facility was observed to be clean, sanitary and in good repair.
Additionally, Licensee is to inform LPA once residents are admitted to the facility, along with proof of active liability insurance.
Exit interview conducted and a copy of the report was provided.
