Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived to the program unannounced to conduct an annual required inspection. LPA Valerio was met by facility staff at the check in area, and was introduced to Program Director (PD)Anthony Boomfield.
LPA Valerio was informed that effective August 1, 2026, Reach Adult Development Inc. will no longer accept clients that require medications. LPA requested an updated plan of operation or addendum to their plan of operation.
LPA Valerio and PD Boomfield toured the program area to ensure compliance with Title 22 regulations. LPA Valerio observed multiple staff on shift. LPA Valerio observed four (4) class rooms, which were observed to be free from hazardous items. Clients were observed engaging in activities and getting ready for snack time. LPA Valerio observed computer areas accessible to clients. The television/projection room was furnished and playing a movie for clients. LPA Valerio observed a quiet room area, which was furnished with a couch for resting. LPA Valerio observed the kitchen area/staff break room area. LPA Valerio observed two fire extinguishers, which were observed to be fully charged with a last service date of May 8, 2026. The program has two bathrooms. The faucet delivered hot water at a temperature of 108.6 degrees Fahrenheit. There were no health or safety concerns observed during today's visit.
LPA Valerio observed client and staff files. Files were observed to be up to date with required documentation. LPA Valerio requested the following be sent to christina.valerio@dss.ca.gov: LIC 500, LIC 308, LIC 610D, and updated copy of plan of operation. Per California Code of Regulations (CCR) - Title 22 - no deficiencies were observed. An exit interview was held, and a copy of this report was provided to PD Boomfield.
