This is an official report of an unannounced visit/investigation of a complaint received in our office on

06/14/2022 and conducted by Evaluator Renee Campbell

COMPLAINT CONTROL NUMBER: 27-AS-20220614152156

On 09/02/2022 at 10:00 AM, Licensing Program Analyst (LPA) R. Campbell and LPA M. Bilger arrived unannounced to investigate the allegation above. LPA's met with Administrator Rebecca Boone and Owner Linda Mustine and explained the purpose of the visit. Upon review of records provided and in person interviews, the Department concludes that there is not enough support to substantiate the allegations that the residents were left unattended. Based on facility floor plan, staff have their own living quarters. LPA’s observed staff were present on earlier visits on 08/03/22 and later on 09/02/22 and were assisting residents as required..

Based on all the information collected by the Department there is not a preponderance of evidence to prove the allegation occurred, therefore this allegation is UNSUBSTANTIATED. Due to the above noted information, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, and therefore the allegations are unsubstantiated.
