Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with Facility Manager Victoria Gomez, and explained the purpose of the visit.
LPA Valerio observed two residents in the living room area while the other two residents were picked up to go to Day Program. LPA Valerio observed two staff on shift, which were observed assisting with daily chores and assisting the residents to get ready for the day. LPA Valerio toured the facility to ensure compliance with Title 22 regulations. LPA observed all areas of the home including both living rooms, dinning room, kitchen area, laundry area, staff/storage area, resident bedrooms, resident bathrooms, closets, and the exterior surrounding areas. All areas of the home were observed to be in good repair, organized, and free from debris. There were no foul odors throughout the home. The facility was observed to be fully stocked in hygiene supplies and food items. There were no health, safety, or personal rights violations observed during the tour.
On 07/23/2025, Shakil's Care Home had their Title 17 Monitoring Review. It was found that Staff 1 (S1) did not have completed CEUs and had the facility complete a corrective action plan that was completed on 07/25/25. LPA Valerio reviewed two (2) staff files and one (1) resident file. Staff files were complete with all training and up to date material. Resident file was complete with required annual documentation.
LPA requested the following documentation be sent to the Regional office: LIC 500, LIC 308, LIC 610D, and Copy of Surety Bond
Per California Code of Regulations (CCR) - Title 22, no deficiencies observed. An exit interview was held with Facility Manager Victoria Gomez, and a copy of the report was provided
