Facility Evaluation Report

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to this facility to conduct a case management visit for the purpose of ensuring Staff 1 (S1), an excluded person, is not present in the facility. LPA Valerio contacted Licensee Phyllis Leggs via cell phone to announce LPA was present at the facility. LPA Valerio was later met by Keisha Leggs, and explained the purpose of the visit.

Staff 1 (S1) is excluded as a result not related to this facility. LPA Valerio did not observe S1 on shift. LPA did not observe S1 on the facility schedule. Licensee was advised to remove S1 from their guardian approved roster and to be aware that S1 is not allowed to be employed and/or on any facility premises.

An exit interview was held, and a copy of this report was provided.
