Complaint Investigation Report
The licensees have not been observe to engage in any inappropriate manner. Staff reported having good rapport with the residents.
LPA Valerio interviewed three (3) residents. R1 stated the staff and owners are good people and they treat them with respect. R1 reported disliking when staff tell R1 what to do, such as clean the room. R2 reported the owners being nice, staff being helpful, and has never felt degraded in any way by staff. R3 reported speaking to another LPA for complaint 27-AS-20240222092835. R3 stated R3 is cool with the owners and they are good to the residents. R3 reported Staff 4 (S4) to have an attitude sometimes; however, S4 is not there often.
According to resident interviews, residents also reported the facility to be clean and are satisfied with the facility. Residents reported to not see any mold in the kitchen and observed the kitchen to be clean.
LPA Valerio observed the facility. LPA Valerio compared the pictures provided by the RP to the facility. The pictures did not match the layout of the facility. The pictures appeared to be from google and did not match the kitchen stove. LPA Valerio did not observed mold in the kitchen and observed the refrigerator to be clean.
According to the RP, the RP stated RP worked consecutive days straight with no relief and was the only person on shift.
LPA Valerio reviewed the facility staff schedule. LPA Valerio observed the staff to resident ratio to be within compliance of the current resident needs. The only time LPA observed one staff member on shift was during 12:00 AM - 6:00 AM. During the day shift and evening shift, there was a minimum of 2 staff on shift. When residents are not at day program or home on weekends, there was a minimum of 3 direct care staff plus the administrator on shift.
Due to the above noted information, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, and therefore the allegations are unsubstantiated. Per California Code of Regulations (CCRs) - Title 22, no deficiencies are being cited. An exit interview was held, and a copy of report was left at the facility.
