Facility Evaluation Report

Alta California Regional Center (ACRC) requested to have meeting with The Regional Office and community partners. On 01/08/2024 at 2:00 PM, an office meeting was held via Microsoft Teams to discuss Hopeful Tomorrow Homes, LLC and how to better serve individuals in care. Present in the meeting were ACRC representatives: Judy Licciardello , Christy Iwasa, Erin Sterling, Joann Callanta, and DeDe Peter; Long Term Care Ombudsman (LTCO) representative Ron Carrerra; Department of Developmental Services (DDS) representatives: Department Chief, Catherine Knight and Behavior Specialist, Amy Stirman; Community Care Licensing (CCL) Representatives: Stephen Richardson and Christina Valerio; and Facility Representatives: Licensee, Marie, and Administrator Monalyn Andrada.

Coordination of Visits to Reduce Multiple Entries/Interactions with Resident 1 (R1)

Continue to collaborate with community partners

Continue to investigate any allegations brought forth to the Department

Collaborate facility visits with community partners, if necessary

Continue to communicate and collaborate with ACRC, DDS, CCL, and LTCO

Per California Code of Regulations (CCR) - Title 22 regulations, no deficiencies are being cited today. A copy of this report will be provided via e-mail to Administrator Monalyn. Administrator to review, sign the hard copy, and submit the signed copy to LPA Valerio.
