Facility Evaluation Report

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to the facility to conduct a case management visit. LPA Valerio met with facility staff, and explained the purpose of the visit. LPA was later met by Administrator Monalyn Andrada.

The Regional Office received a Facility Action Report (FAR) dated 09/10/2024 from Alta California Regional Center. This FAR was a follow up visit for their quarterly quality assurance audit on 08/27/2024. Please refer to CCL visit date 04/15/2024. Based on FAR dated 09/10/2024, staff did not pass the RBT certification test. As of today’s date, 21 Direct Support Professional currently work at the facility and do not have their Registered Behavioral Technician Certification. ACRC has implemented a corrective action place (see below) that will ensure all staff have completed RBT certification by February 28 th , 2025.

• By December 31, 2024, 40% of staff will have completed RBT certification.

• By January 31, 2025, 60% of staff will have completed RBT certification.

• By February 28, 2025, 100% of staff will have completed RBT certification. "

Per California Code of Regulations (CCR) – deficiencies are being cited today on the attached LIC 809 – D page. An exit interview was held, and a copy of the report was provided. Appeal rights were provided.
