Facility Evaluation Report

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct a case management visit. LPA met with Administrator Monalyn, and explained the purpose of the visit.

LPA observed the facility to be clean and free from debris. LPA observed multiple staff on shift that met staffing ratio requirements. No immediate health or safety concerns observed.

The Regional Office received a Facility Action Report from Alta California Regional Center (ACRC) dated 03/13/2024. The facility was observed by ACRC to be in violation of Title 17 Section 56054(a). There were 9 Direct Support staff who have not received Registered Behavioral Technician Certification within one year of hire dates per Title 17 Regulation Section 59055 Direct Care Staff Qualifications.

Per California Code of Regulations (CCR) - Title 22, Division 6, Chapter 11, the facility is being cited today on the attached LIC 809-D page. Appeal Rights were provided. An exit interview was held, and a copy of the report was provided.
