Facility Evaluation Report
Licensing Program Analayst (LPA) Christina Valerio arrived unannounced to conduct the annual required inspection. LPA Valerio met with Administrator Monalyn, and explained the purpose of the visit.
LPA Valerio and Administrator Monalyn toured the facility to ensure compliance with Title 22 regulations. The common living room area was observed to have furniture for day lounging or watching television, a bike to engage in indoor exercise, multiple indoor games, and other indoor activities. The kitchen was observed to be clean and sanitary. The facility was observed to have an adequate food supply. Food pantry and refrigerators were observed to be accessible to residents at all times. Resident bedrooms were observed to be fully furnished and personalized. Resident bathrooms were observed to be fully stocked with hygiene supplies and in sanitary condition. Exterior areas were observed to have an large open area for outdoor seating and an area to play basketball. The staff area was observed to be utilized for administrative storage and staff break room. Medications, sharps, and hazardous items were observed to be locked and inaccessible to residents in care. Multiple staff were observed to be on shift. Staff were assisting residents with morning daily routines, cleaning, and assisting with administrative tasks. The fire extinguisher and fire sprinklers were last serviced in January of 2025. The date of the last fire drill October 08, 2025.
LPA Valerio reviewed two (2) staff and two (2) resident files. All files were observed to up to date with required annual documents. LPA Valerio requested the following be sent via email: LIC 500, LIC 308, LIC 610, and copy of surety bond
No deficiencies are being cited. An exit interview was held with Administrator Monalyn, and a copy of this report was provided.
