The inspector’s account

LPA Valerio reviewed training documentation for years 2023 and 2024. LPA did not observe the RP's name on any training or staff schedules for year 2023 and year 2024. For 2023, Ukeru training was provided in August of 2023 and December of 2023. 23 staff were in attendance for the August 2023 training and 28 staff were in attendance for the December 2023 training. For 2024, Ukeru pad training was conducted on 06/05/2024 with 26 staff member in attendance. According to Administrator Monalyn, the facility does not have staff participate in Pro-Act training, instead they require staff to participate in Crisis Prevention Institute (CPI) training.

In addition to the Ukeru training, staff were provided monthly training for various topics for 2024. A list of topics are listed below:

January - Addressing Stress in Workplace, Resident Medication List Update, Resident IBSP, ISP, IBSP, IEIP, and 20 minute refresher on Bite Release

February - Importance of Client Privacy, Resident Medication List Update, Resident ISP, IBSP, and IEIP, 20 minute refresh on WRIST (lock n key, block n key, pull the lever)

March - Implementing Effective Check-Ins with Client with Disabilities, Resident Medication List Update, Resident ISP, IBSP, and IEIP, and 20 minute refresher Physical intervention: Disengagement Seated Position

April - Providing First Aid to an Adult with Disabilities, Resident Medication List Update, Resident ISP, IBSP, and IEIP, 20 minute refresh on Physical Intervention: Disengagement Neck: Lower- Medium-Higher"

May - Key Components on how to support individuals with disabilities and behaviors, Resident Medication List Update, Resident ISP, IBSP, and IEIP, and 20 minute refresher on grabs/hold - maintaining physical contact without consent "WRIST/ARM"

June - Caring for Clients with Self Injurious Behavior, Resident Medication List Update, Resident ISP, IBSP, and IEIP, 20 minute refresher - Physical Intervention - Disengagement skills - Neck - Choking

It is to be noted that the facility was cited on 04/15/2024 for having 9 direct care staff that did not become a Registered Behavior Technician within twelve (12) months of initial employment. Per Title 17 and Title 22 regulations, the facility was required to ensure all staff have met this requirement. The facility has cleared there plan of correction and has the staff enrolled to take their RBT test August 2024.

Due to the above noted information, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, and therefore the allegations are unsubstantiated. Per California Code of Regulations (CCRs) - Title 22, no deficiencies cited. An exit interview was held, and a copy of report was given to Administrator Monalyn.
