Facility Evaluation Report

Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct a case management visit. LPA Moleski spoke with house manager Brittany Chaves over the phone and explained the purpose of the visit. Chaves said that staff member Mohammed Lughmani could sign this report in her absence.

LPA Moleski returned documents previously seized on 2/13/25 as part of a complaint investigation pursuant to 22 CCR Section 80070(d). LPA Moleski had previously informed Chaves that he would be returning these documents by 2/19/25, as required by Section 80070(d)(3).

An exit interview was held and a copy of this report was left with Lughmani.
