The inspector’s account

On 09/11/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the day program to conduct an annual inspection. Upon arrival, LPA Campbell observed two separate rooms for clients. Galt II provides care at a ratio of 1 staff to 2 clients. Galt I next door, provides care at a ratio of 1 staff to 3 clients. LPA Campbell first met with Victoria Pfaller, Trainer for Galt II and Malcolm Fowlkes, Site Coordinator in Galt I. LPA Campbell, toured the space and requested files. LPA Campbell then went next door to do same.

The day program is housed in two rooms in a single story building for the developmentally disabled and is licensed to serve 61 adults/elderly clients who are 18 and up. Suite 116 may serve 26 clients with 5 being non-ambulatory. Suite 117 has a capacity of 35 clients, 15 of whom may be non-ambulatory.

During the tour, LPA Campbell observed, hot water measurements, thermostat settings, activiity supplies,the activity calendar. Toxic supplies were observed to be locked up along with sharps and inaccessible to clients in care. Hot water in the two bathrooms were measured at 117 degrees Fahrenheit (F) and 119 degrees F. Hot water in the kitchen was observed at 117 degrees F and within the required temperatures of 105 to 120 degrees F.

Galt II has 11 staff and Galt I has 8 staff for a combined total of 19 staff. LPA Campbell reviewed 3 files and found them to be complete. Three client files were reviewed and also were complete. All staff were confirmed as being associated to the day program location. No deficiencies were observed or cited during this visit. Exit interview held and a copy of report was left .
