Facility Evaluation Report

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required visit. According to Licensee Jessica Bowen, there are zero (0) residents in care. On 10/16/2025, LPA spoke to Licensee Jessica regarding their situation with the landlord and a possible change of location. On 10/17/2025, LPA Valerio arrived to confirm no residents in care/conduct annual visit. There were no persons on the premises and it appeared that all windows were closed. There were also no visible cars in front of the home.

On 11/05/2025, LPA Valerio spoke to Licensee Jessica Bowen via cell phone to discuss a change of location and proof of control of property.

On 11/24/2025, Licensee Jessica Bowen stated they do not have control of property.

Due to the above information, the licensee has lost control of property and therefore, the license is no longer valid and has been forfeited.

Licensee Jessica stated she cannot due a walk through and does not have access to obtain the original copy of the license.

This letter will be mailed certified to the licensee's address on file.
