Facility Evaluation Report
Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with co-Administrator and explained the purpose of the visit.
LPA and co-Administrator conducted a tour of the interior and exterior of the facility to ensure the health and safety of residents in care. Areas toured included but not limited to: residents rooms, bathroom, kitchen, backyard and the common areas. LPA observed facility to have at least two days of perishable and seven days of nonperishable foods. LPA observed sharps to be locked and secured. LPA observed medications to be locked and secured. LPA observed fire extinguisher present with service date of August 25, 2025. LPA observed facility to be clean, safe, sanitary and in good repair. LPA and co-Administrator discussed no concerns at the facility at this time.
LPA observed Emergency And Disaster Plan For Residential Care Facilities For The Elderly LIC610E, document recently reviewed by Administrator on June 16, 2025. LPA conducted a file review for six residents records via facility laptop, and confirmed the required documents were present.
CARE tool was completed and found facility to be in compliance.
Result of today's visit, no deficiencies observed.
Exit interview and a copy of the report was provided.
