Facility Evaluation Report

Licensing Program Analyst (LPA) Angela Hood arrived at the care home today and met with the Executive Director, Liz Cruz, to conduct a case management visit. The purpose of today's visit was to amend reports from a visit conducted on February 3, 2026.

The department has determined that, due to the citation issued on February 3, 2026, an immediate civil penalty per Health and Safety Code § 1548 in the amount of $500 will be assessed for a violation that the department determines resulted in the injury or illness of a person in care. An additional civil penalty assessment is under review and a determination is pending. LPA will return on a future date to assess an additional civil penalty if warranted.
