Staff did not respond to resident in a timely manner.
On 7/13/2026, Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Administrator Arcely Pua.
During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation.The results of the investigation are as follows: Based on interviews conducted, there is insufficient evidence to support the allegation. Staff reported that emergency services arrived at the facility door at approximately 11:46 p.m. and staff were awakened by knocking at the door. Staff did not place the call for emergency services. Staff reported that it took approximately five minutes to determine what was occurring and to ensure the safety of residents before opening the door due to the late hour. Based on the information obtained, staff responded within a reasonable amount of time after being alerted to the situation. No evidence was found indicating that staff failed to respond to a resident’s request for assistance or delayed responding to a resident’s immediate needs. Therefore, the allegation is determined to be unsubstantiated. A finding that the complaint is UNSUBSTANTIATED means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Exit interview conducted. Report left with facility.
