Facility Evaluation Report

Licensing Program Analyst (LPA) Michael Hood arrived at the facility and met with caregiver, Angela Campbell, to follow-up regarding deficiencies and technical assistance issued at the care home during inspection conducted on December 16, 2025.

LPA reviewed staff training during inspection and provided technical assistance. LPA verified a certified Administrator. LPA will clear deficiency once they receive requested documentation to assign certified Administrator. LPA reviewed resident records and provided technical assistance regarding admission agreements.

No deficiencies are being cited as a result of today's visit. Exit interview was conducted. Signature on these forms acknowledges receipt of these documents.
