Facility Evaluation Report
On January 6, 2026, Licensing Program Analyst (LPA) Michael Hood met with Facility Representative Pouya Ansari for an unannounced inspection to follow-up on substantiated complaints.
On August 29, 2023, the Department concluded a complaint investigation regarding the following allegations: Resident sustained pressure injuries while in care and the facility did not seek timely medical care for pressure injuries. The licensee was cited for California Code of Regulations (CCR) Title 22, § 87466 Observation of Resident, and CCR Title 22, §87465(a)(1) Incidental Medical and Dental Care.
On August 29, 2023, the issuance of an immediate civil penalty for $500 was issued and Licensee was informed that a civil penalty per Health and Safety Code § 1569.49(f) is under review, and a determination is pending.
The Department has concluded an analysis and has determined that a civil penalty is warranted for serious bodily injury. The Welfare and Institutions Code Section 15610.67 defines serious bodily injury as “injury involving extreme physical pain, substantial risk of death, or protracted loss or impairment of function of a bodily member, organ, or of mental faculty, or requiring medical intervention, including, but not limited to, hospitalization, surgery, or physical rehabilitation.” This is evidenced by the facility not providing proper care and supervision resulting in a resident (R1) being diagnosed with multiple unstageable pressure injuries that required hospitalization, wound care, and antibiotics.
