Facility Evaluation Report

Licensing Program Analyst (LPA) Michael Hood and Licensing Program Manager (LPM) Lauren Crocker arrived at the care home unannounced on June 16, 2026 to conduct a Required-1 Year Inspection utilizing the inspection tool. LPA and LPM met with Volunteer, Trisha Woodward, and observed volunteer on the premises as the sole staff member on site.

LPA and LPM conducted an inspection of the care home to ensure compliance with Title 22 regulations. LPA and LPM will return to the care home at a later time to conduct an annual continuation and complete annual inspection.

As a result of this visit, no deficiencies were cited per California Code of Regulations, Title 22. Exit was interview conducted and copy of report given at the conclusion of this visit.
