Facility Evaluation Report

Licensing Program Analyst (LPA) Michael Hood arrived at the facility and met with Administrator, Cleopatra Grant, to conduct a POC visit regarding citations issued on March 20, 2025.

During visit, LPA reviewed staff member (S1's) training and observed initial training to meet the Health and Safety Code requirements. LPA also obtained a statement of understanding regarding quarterly drills and reviewed documents on cite regarding quarterly drills. Deficiencies were cleared during inspection.

LPA will request another fire inspection due to previous request not having appropriate contact information.

No deficiencies were cited during inspection. Exit interview was conducted. A copy of this report was provided. Signature on these forms acknowledges receipt of these documents.
