Facility Evaluation Report

On May 6, 2026, Licensing Program Manager (LPM) Lauren Crocker and Licensing Program Analyst (LPA) Michael Hood arrived unannounced at the facility to deliver a Temporary Suspension Order (TSO). The TSO shall be effective IMMEDIATELY. The Regional Office staff were met by caregiver, Isaiah Davis, upon arrival.

Licensee, Mark Graham, was not available during today's visit. A call was made to the Licensee by LPA, LPM, and from the facility phone. Calls did not ring and were sent straight to voice message. Voice mailbox was full and no calls were able to leave a voice message.

LPM and LPA conducted a tour of the facility and observed that there are currently no residents residing at the care home. LPM reviewed the TSO paperwork and left copies along with appeal rights.
