Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Hood arrived at the facility and met with House Manager, Stephanie Harmon-Christensen, to conduct a case management visit to follow-up regarding information the Department obtained on April 17, 2026.
LPA received information that Licensee was served an Unlawful Detainer Complaint on April 19, 2026. As of April 17, 2026, all necessary parties have been notified of Unlawful Detainer Complaint. On April 10, 2026, the facility was given a Final Notice that electric service was about to be turned off. The date that the power would be shut off was April 16, 2026. Interview with Licensee indicated that the electric bill was paid. LPA observed that electricity was not shut off as of April 20, 2026. Licensee has yet to provide notices to all necessary parties regarding Final Notice that electric services were to be turned off.
The facility currently does not have a certified administrator. A civil penalty in the amount of $600 was assessed for today's date due to failure to correct a citation regarding obtaining a certified administrator.
As a resu lt of today's inspectio n, a deficiency is being cited pursuant to Health and Safety Code Section § 1569.686 due to Licensee not issuing notices to all necessary parties regarding Final Notice that electric services were to be turned off. Deficiency is listed on 809-D. A licensee who does not comply with this section may be liable for civil penalties in an amount not to exceed one hundred dollars ($100) for each day the facility did not provide notification required in Health and Safety Code Section §1569.686. A civil penalty in the amount of $800 was issued today per Health and Safety Code Section §1569.686.
