Facility Evaluation Report

Licensing Program Analyst (LPA) Michael Hood and Licensing Program Manager (LPM) Lauren Crocker arrived at the facility and met with House Manager, Stephanie Harmon-Christensen, to conduct a health and safety check. During visit, LPA and LPM spoke with Licensee, Mark Graham, via telephone call to discuss concerns regarding financial solvency.

The facility currently does not have a certified administrator. A civil penalty in the amount of $500 was assessed for today's date due to failure to correct a citation regarding obtaining a certified administrator.

No deficiencies are being cited as a result of today's visit. Exit interview was conducted. Signature on these forms acknowledges receipt of these documents.
