On 01/21/2026 Licensing Program Manager (LPM) Troy Ordonez, Laura Munoz and Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct health and safety check met with house manager Stephanie Harmon-Christensen.
Five (5) resident and two (2) staff present at the facility. During today's visit LPM and LPA reviewed resident records and obtained a copy of staff schedule.
No deficiencies cited during today's visit.
Exit interview conducted and a copy of the report was left at the facility.
