Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Hood and Licensing Program Manager (LPM) Lauren Crocker arrived at the facility and met with Licensee, Mark Graham, to address and discuss concerns regarding the most recent inspection conducted on August 18, 2025.
During visit, LPA and LPM discussed with Licensee the importance of obtaining a criminal background clearance for volunteers prior to them providing supervision to the residents in care. LPA and LPM also discussed unpaid license fees. Licensee agrees to pay all license fees within 60 days of this report.
Technical support was offered to facility representative during visit.
As a result of today's inspection, a deficiency is being cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. A civil penalty in the amount of $100 is being assessed today due a one (1) volunteer not receiving a criminal background clearance. Deficiency is listed on 809-D.
