Licensing Program Analyst (LPA) Michael Hood met with caregiver, Yolanda Bishop, to conduct a case management health and safety check following an inspection conducted on 10/24/2023. LPA spoke with Administrator, Angie Crudo, via telephone call to discuss visit. Administrator gave permission for caregiver to sign report.
During visit, LPA toured the facility and discussed requested documents with Administrator.
No deficiencies are being cited as a result of today's inspection.
An exit interview was conducted with the Administrator. A copy of this report was provided to the facility. Signature acknowledges receipt of this report.
