Facility Evaluation Report
Licensing Program Analysts (LPAs) Kevin Mknelly and Michael Hood arrived at the facility and met with Administrator, Angelina Crudo, to conduct an inspection.
During today's visit, LPAs observed multiple hazardous items unlocked and accessible to residents in the kitchen area when staff were not in the kitchen area, including knives and disinfectants. LPAs observed centrally stored medications unlocked in the kitchen area. LPAs also observed multiple expired food items in the kitchen area.
As a result of today's inspection, deficiencies are being cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. Deficiencies are listed on attached 809-D.
