Facility Evaluation Report

Licensing Program Analyst (LPA) Michael Hood and Licensing Program Manager (LPA) Lauren Crocker arrived at the facility and met with Licensee, Angelina Crudo, to conduct a health and safety check case management visit.

LPA and LPM spoke with Licensee on the status of residents' relocation. Five (5) residents will be transferred to a new location with the help of their insurance. One (1) resident is receiving help with relocation from family. Licensee ensures that all residents will be relocated upon closure of the license. LPA and LPM reviewed all resident records during visit.

As a result of this visit, no deficiencies were cited. Exit interview was conducted. A copy of this report was provided. Signature on these forms acknowledges receipt of these documents.
