Facility Evaluation Report

Licensing Program Analyst (LPA) Michael Hood arrived at the facility and met with caregiver, Sonia McComic, to conduct a health and safety check case management visit. LPA spoke with Licensee, Angie Crudo, via telephone call, who gave permission to have caregiver sign report.

LPA spoke with Licensee on the status of residents' relocation. Residents will be transferred to a new location with the help of their insurance.

As a result of this visit, no deficiencies were cited. Exit interview was conducted. A copy of this report was provided. Signature on these forms acknowledges receipt of these documents.
