Licensing Program Analyst (LPA) Michael Hood arrived at the facility and met with Licensee, Angelina Crudo, to conduct a health and safety check case management visit.
LPA spoke with Licensee on the status of residents' relocation. Residents will be transferred to a new location with the help of their insurance. LPA received 60-day notices issued to the residents prior to the visit.
As a result of this visit, no deficiencies were cited. Exit interview was conducted. A copy of this report was provided. Signature on these forms acknowledges receipt of these documents.
