Facility Evaluation Report

An office meeting was conducted at 10:00am on April 28, 2025, with Sacramento North Regional Office via Microsoft Teams. The purpose of this meeting was to review and discuss stipulation and waiver and order ordered on April 22, 2025.

Plan for facility closure effective June 21, 2025

Licensee was notified that the Department may increase monitoring of the facility prior to facility closure of June 21, 2025.

No deficiencies were cited. An exit interview was conducted and a copy of this report will be provided to the facility via email. A copy must be signed and returned to the Department.
