Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Hood arrived at the facility and met with Licensee, Jeanina Lita, to follow-up regarding evidence received during a separate inspection conducted on May 29, 2025.
LPA obtained a "3-Day Notice to Pay Rent or Vacate" that was issued to resident (R1) on March 5, 2025 and signed by Licensee, Jeanina Lup. The notice states, "PLEASE TAKE FURTHER NOTICE that with THREE (3) days after service of this notice, excluding Saturdays, Sundays, and court holidays, you are hereby required to pay the above-listed amount in full OR vacate the subject premises, move out, and deliver up possession of the same to Jeanina Lup. Failure to pay the rent in full OR vacate the premises WITHIN THREE (3) days as required by this notice will result in forfeiture of the lease and/or rental agreement and will institute legal proceedings for an unlawful detainer against you to declare the lease or rental agreement void or forfeited, recover rent, damages, and possession of said premises." LPA did not receive a copy of the notice prior to being issued to R1. LPA determines, after review, that the notice is a violation of R1's personal rights.
As a result of today's inspection, a deficiency is being cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. Deficiency is listed on 809-D pages.
