Facility Evaluation Report
Licensing Program Analyst (LPA) Hiratsuka arrived at the facility unannounced to conduct a required annual inspection. LPA met and toured the facility with Administrator Halima Mbarack.
A tour was conducted of the interior and exterior of the facility. Areas toured included but not limited to: four private clients room, bathrooms, laundry room, kitchen, utility room, medication room, staff office, the detached office and the backyard.
LPA observed bathrooms to be clean, sanitary and odorless and consisted of grab bars and non-skid mats. LPA observed facility to have a sufficient supply of linens, towels, bedding for clients in care. Washer and dryer was present. Bedroom was observed to have appropriate furniture for clients. Facility had ample perishable and nonperishable food. No immediate health, safety and/or personal rights violation was observed.
Three resident records were reviewed. Three staff records were reviewed.
