Facility Evaluation Report
Licensing Program Analyst (LPA) Cassie Yang arrived at the facility announced to conduct a Pre-Licensing inspection utilizing the inspection tool. LPA met with Applicant and explained the purpose of the visit.
LPA and Applicant conducted a tour of the interior and exterior of the facility. Areas toured included but not limited to: four private clients room, bathrooms, laundry room, kitchen, utility room, medication room, staff office, the detached office and the backyard.
LPA observed bathrooms to be clean, sanitary and odorless and consisted of grab bars and non-skid mats. LPA observed facility to have a sufficient supply of linens, towels, bedding for potential clients in care. Washer and dryer was present. Bedroom was observed to have appropriate furniture for clients. No immediate health, safety and/or personal rights violation was observed.
Comp III was waived as Applicant has multiple facilities with Community Care Licensing and is aware of reporting requirements.
Licensure is pending approval from Central Application Bureau (CAB). CAB analyst will contact Applicant with a copy of license once approved.
