Facility Evaluation Report

On December 11, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding an email LPA received on December 10, 2025. LPA met with Administrator and explained the purpose of the visit.

Today's visit, LPA and Administrator discussed facility's plan for closure. Administrator informed LPA that facility closure 180 day notice was effective as of December 10, 2025. LPA was informed that the facility is keeping constant communication with Alta California Regional Center. Facility may seek facility closure and/or possibly change of ownership.

LPA informed Administrator that an office meeting may be held with Licensee to discuss the proper steps in closure and/or change of ownership. Administrator stated updates will be provided if it is confirmed for change of ownership.
