The inspector’s account

On October 28, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to discuss an incident report that was submitted to LPA on October 20, 2025. LPA met with Behavioral Consultant and explained the purpose of the visit.

The incident report stated incident occurred on October 19, 2025, which client (C1) alleged that staff had forced C1 to intake their medication but putting the pill in C1's mouth and tilting C1's head back to swallow. Internal investigation was conducted and found that staff denied the incident, additionally, witnesses at the facility denied observing the following. Interview was further conducted with C1 which C1 then stated this incident did not happened.

Today's visit, LPA spoke with Behavioral Consultant regarding the incident and was informed that C1's behaviors was declining. However, there has been a subtle change in C1's living situation which caused C1 to acting out. LPA was further informed that C1 wanted to take the medication later but due to C1's condition, staff encouraged C1 that it is best to intake in medication immediately which causes C1 to be upset. LPA was informed further client consultations will be conducted to ensure needs are being met.

Facility reported no current concerns at the facility at this time. No deficiencies cited.

Exit interview conducted and a copy of the report was provided.
