The inspector’s account

Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct an annual inspection. LPA Moleski met with director Kathy Rhymes and explained the purpose of the visit.

LPA Moleski reviewed two client files (C1-C2) and two staff files (S1-S2). LPA Moleski requested S3's file, but none could be provided. LPA Moleski observed that S3 did not have a criminal record clearance on file. According to Rhymes, S3 is 16 years old and performs cleaning duties, but does not provide care for clients. Staff members below the age of 18 are allowed to work in non-care classifications, and are not required to have a criminal record clearance on file. However, S3 does still need a personnel file on the premises.

LPA Moleski reviewed Guardian records and observed that both S1 and S2 do have criminal record clearances, but neither were associated to this facility's roster at the time of this visit. According to Guardian records, S1 was never associated to this facility, while S2 was separated from the facility roster in February 2015. Rhymes associated both S1 and S2 during this visit. However, neither S1 nor S2 had first aid/CPR certificates on file. S2 also had no health screening on file.

LPA Moleski toured the facility with Rhymes and inspected common areas, the kitchen, classrooms, and bathrooms. Furniture and furnishings were sufficient to meet the needs of clients. The facility temperature was 70 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 116 degrees Fahrenheit, which is within the required range of 105 and 120 degrees.
